Describes how to complete the Facility Survey in MHPD in preparation for reporting client data in the Patient Characteristics Survey in the fall of 2015. In the Facility Survey, providers update their list of licensed or OMH-funded programs by submitting requests to close or open programs. The Webcast describes how existing MHPD users may self-register in MHPD for NY.gov User IDs.
