New York State Clinical Records Initiative (NYSCRI)

NYSCRI is the New York State Clinical Records Initiative. The Office of Mental Health (OMH) and the New York State Office of Alcoholism and Substance Abuse Services (OASAS) offer this standardized, compliant and integrated clinical case records form set, on a voluntary basis, for their local licensed non-inpatient programs. NYSCRI was initiated in 2009 through a collaborative effort of the OMH Long Island Field Office, Office of Alcoholism and Substance Abuse Services (OASAS) Regional Office, Nassau and Suffolk County Divisions of Mental Hygiene and the Long Island Coalition of Behavioral Health Providers. A representative group of providers, state/county agency staff, as well as recipients and families, worked to develop the form set. First piloted on Long Island, the forms are now available to providers statewide in this extended pilot project.

Electronic Medical Records vendors and interested providers/entities wishing to obtain NYSCRI Certification may obtain further information on fees, the application process and other details by contacting Scott Lloyd, President, MTM Services. Once certified, OMH will list these vendors on our website. Certification is performed on behalf of OMH and OASAS by MTM Services. This process assures providers the vendor's program contains all the NYSCRI data elements and linkages.
