New York State Clinical Records Initiative (NYSCRI)
New York State Clinical Records Initiative logo

NYSCRI is the New York State Clinical Records Initiative. The Office of Mental Health (OMH) and the New York State Office of Alcoholism and Substance Abuse Services (OASAS) offer this standardized, compliant and integrated clinical case records form set, on a voluntary basis, for their local licensed non-inpatient programs. Access the form set. The Form Usage index Excell Document is provided for reference.

NYSCRI was initiated in 2009 through a collaborative effort of the OMH Long Island Field Office, Office of Alcoholism and Substance Abuse Services (OASAS) Regional Office, Nassau and Suffolk County Local Governmental Units (LGUs) and the Long Island Coalition of Behavioral Health Providers. A representative group of providers, state/county agency staff, as well as recipients and families, worked to develop the form set. First piloted on Long Island, the forms are now available to providers statewide.

Further resources are available, including the NYSCRI Manual; Compliance Grid; Form Element Glossary, access to a NYSCRI Listserv; OMH Diagnosis Waiver; and NYSCRI guidance webinars.
