1999 Patient Characteristics Survey
The 1999 Patient Characteristics Survey form and instructions are available in Portable Document Format (PDF). Software called Adobe Acrobat Reader is needed to read or print PDF documents. You may download a free Acrobat Reader from Adobe or call 1(800) 430-3586 to order a printed copy of the documents.
You can select a number of reports that reflect results of the 1999 PCS. Information is summarized at three levels - county (of provider), region (of provider), and state. Each level contains seven reports:
- Major Age Group by Program: Number of people served in each age group (Under 18 years, 18 to 64 years, 65 years and older) broken down by approximately 80 service programs.
- Ethnicity by Program: Number of people served in each ethnic group broken down by approximately 80 service programs.
- SPMI/SED Status by Program: Number of people served who were SPMI/SED, and the number who were not SPMI/SED, broken down by approximately 80 service programs.
- Reported Residence by Program: Number of people served in each residence type broken down by approximately 80 service programs.
- Detailed Age Group by Gender By Ethnicity: Number of people served in each age group (Less than 5 years, 5-12 years, 13-17 years, 18-20 years, 21-34 years, 35-44 years, 45-64 years, 65-74 years, 75 years or older) broken down by gender and ethnic group.
- Diagnosis Category by Various Criteria: Number of people served in each diagnosis category by: current disabilities, prior services, SMI/SED status, SSDI/SSDI enrollment, and medicaid enrollment.
- Diagnosis Category by Source of Referral: Number of people served in each diagnosis category by source of referral.
