Tools for Managing the Continuous Quality Improvement Project
The Psychiatric Services and Clinical Knowledge Enhancement System (PSYCKES) team has developed an array of project tools for hospital Quality Improvement (QI) teams to use in managing their Continuous Quality Improvement (CQI) projects.

Action Plan
A written Action Plan
(300kb) is an important tool for planning and managing the CQI project. The action plan documents interventions to be implemented and strategies for tracking project activities and outcomes. The action plan includes the following:
- Project goals
- Measurable objectives
- Interventions to be implemented
- Individual(s) responsible
- Resources required
- Time frames
- Performance Measures
The QI team revises the action plan periodically, based upon review of project data.
Using a Master Spreadsheet to Manage Project Data
Data-driven decision-making is a key element of successful quality improvement. Therefore, it is essential that QI teams develop effective processes for tracking data regarding project activities and outcomes. The PSYCKES team strongly recommends that each QI team maintain a Master Spreadsheet as the primary vehicle for tracking project data. The spreadsheet should be organized as a list of clients screened and/or identified as being at high risk of readmission. QI teams can include any type of data in the spreadsheet, but most importantly, it should include all data needed for monthly reporting.

These are offered as tools that may be helpful to QI teams in managing the project. Teams may choose to download and enter their data into this spreadsheet, modify it, simply look at it for ideas, or not use it at all. It is not required and is not intended to be submitted to PSYCKES.
