Consolidated Fiscal Reporting System (CFRS)

The CFRS upload page allows you to upload completed CFRS submissions as well as Financial Statements.

Uploading your Completed CFRS Submission to the CFRS web site

Remember, your submission must be Prepared for Upload, prior to Uploading.

After ensuring that step has been completed, then:

  1. Do one of the following:
    • In CFRS, click Help -> CFR Training / Software on the Web. Click Yes. This will bring you to the CFRS Home Page. Click the "Upload" link.
    • In CFRS, on the Prepare Submission for Upload screen, click the "Go to the CFRS Upload web page" button. Click Yes.
    • Open your Web browser to the CFRS Home Page and click the "Upload" link.

  2. click the "Upload Your CFR Submission or financial statements" link. The CFRS Upload page displays.
  3. Type in the "Provider Agency Code" and hit Enter, or click the "Check" button at the bottom of the page. The Provider Confirmation information displays.
  4. If the provider code was not found, check that the code is correct and click Check. If the provider code is correct but is not found, call the appropriate State Agency (see below) to verify the correct code.
  5. If the information displayed is not correct, either click the "Reset" button and type in the correct Provider Agency Code or call the appropriate State Agency (see below) to verify the correct code.
  6. Select "Completed CFR..." for the Type of document to upload.
  7. If you are submitting a CFR for a period for which you have already submitted, check the box next to "This is a re-submit..."
  8. Optionally check the box next to "Send me a confirmation..." to have the system send you an email with the status of the upload. Enter your email address in the box.
  9. Click the "Browse" button, navigate to the location of the file you want to upload. The filename will include your Provider Code, the submission type, the State Agency, your CFR software version, and the file creation date and time, followed by .mdb.cfr   For example, for Version 24.0:
    • 99999-Full-MULTI-[24]-2014C-52967352-20150215173025.mdb.cfr
    Note: If your CFRS software was installed to the default location, your upload file will be located in the following directory dependant on your software's version (Version 24.0 in example):
    • on Windows XP: C:\Program Files\Consolidated Fiscal Reporting System\CFRS 24.0\DB\Upload
    • on Windows Vista, Windows 7, Windows 8: C:\Program Data\Consolidated Fiscal Reporting System\CFRS 24.0\DB\Upload
  10. Click the "Upload" button. The progress and status of the upload is displayed at the bottom of the page. When the upload is complete, a message displays on the Web page, "File uploaded successfully. DCN: 52967352."
  11. If you have another CFR file to upload, select the type of file to upload, . Repeat steps 3 through 10.
  12. Remember to print the Web page as a confirmation that the submission was uploaded successfully, click the Print button. This is your only written confirmation of your successful upload.

    Always create a backup of your CFRS database, especially after you successfully upload and submit your data.

    • SED: 518-474-3227

Uploading your Financial Statements to the CFRS web site

  1. Do one of the following:
    • In CFRS, click Help -> CFR Training / Software on the Web. Click Yes. This will bring you to the CFRS Home Page. Click the "Upload" link.
    • In CFRS, on the Prepare Submission for Upload screen, click the "Go to the CFRS Upload web page" button. Click Yes.
    • Open your Web browser to the CFRS Home Page and click the "Upload" link.

  2. click the "Upload Your CFR Submission or financial statements" link. The CFRS Upload page displays.
  3. Type in the "Provider Agency Code" and hit Enter, or click the "Check" button at the bottom of the page. The Provider Confirmation information displays.
  4. If the provider code was not found, check that the code is correct and click Check. If the provider code is correct but is not found, call the appropriate State Agency (see below) to verify the correct code.
  5. If the information displayed is not correct, either click the "Reset" button and type in the correct Provider Agency Code or call the appropriate State Agency (see below) to verify the correct code.
  6. Select "Financial Statements..." for the Type of document to upload.
  7. If you are submitting financial statements for a period for which you have already submitted, check the box next to "This is a re-submit..."
  8. Optionally check the box next to " Send me a confirmation..." to have the system send you an email with the status of the upload. Enter your email address in the box.
  9. Click the "Browse" button, navigate to the location of the file you want to upload.
  10. Click the "Upload" button. The progress and status of the upload is displayed at the bottom of the page. When the upload is complete, a message displays on the Web page, "File uploaded successfully."
  11. If you have another financial statements file to upload, Repeat steps 3 through 10.
  12. Remember to print the Web page as a confirmation that the submission was uploaded successfully, click the Print button. This is your only written confirmation of your successful upload.
