Project Reporting

Monthly Data Submission Process

Hospitals participating in the Readmissions Quality Collaborative report monthly according to a Reporting Plan developed by the Steering Committee.

An online data submission survey is conducted each month. While the survey is open, the link to the survey is posted on the
Hospital Quality Collaborative page of the Psychiatric Services and Clinical Knowledge Enhancement System (PSYCKES) website and is e-mailed to Readmissions Project Team. The survey opens on or about the 5th of the month, and data is due on the 10th of the month (or the next business day) for the previous calendar month's activities. A separate survey is completed for each participating service.

If an error is found after data is submitted, hospitals should not complete an additional survey. Instead, they should request a correction form by contacting PSYCKES Help.

The PSYCKES team aggregates this monthly self-report data and reports back to the Quality Collaborative.

Phase 2 Monthly Data Reporting Questions

Hospitals and health homes report on project implementation milestones until all milestones have been achieved.  Each hospital or non-hospital affiliated health home should complete a single survey.

In June 2015, hospitals and health homes began tracking and reporting project interventions and initial outcomes. Each participating service and setting reports separately, by completing the monthly data submission survey. Survey questions for each setting and detailed instruction are in the document below:
