Tools for Managing the Quality Improvement Project

The Psychiatric Services and Clinical Knowledge Enhancement System (PSYCKES) team has developed an array of tools for hospital teams to use in managing their Quality Improvement (QI) projects.

Action Plan

A written Action Plan is an important tool for planning and managing the QI project. It should be revised periodically, based on project data.

Managing Project Data

Data-driven decision-making is a key element of successful quality improvement. Therefore, it is essential that QI teams develop effective processes for tracking data regarding project activities and outcomes. The tools below are designed to capture the data needed for monthly reporting. They are entirely optional and may be edited/adapted as needed. Hospital teams should consider whether it would be helpful to capture additional data elements.
