Implementation of Healthy Steps

The Office of Mental Health announces this Request for Proposals (RFP) for the Implementation of Healthy Steps for Young Children into nineteen primary medical care practices serving infants and toddlers from birth to age five across New York State. This intervention represents an approach to primary health care that offers families enhanced well-child visits emphasizing the promotion of children's development and parental support/services.

April 14, 2016 – Correction to the Scoring Points:

Please be advised that in Section 4.1.2 Financial Assessment (page 11), there was a mathematical error in the Scoring points to be used when evaluating the budgets submitted with the proposals. For the Financial Assessment, the Maximum Points to be awarded will be 25 points.

The total Maximum Points to be awarded for proposals will be 130 points.

April 7, 2016 - Correction to Q & A Document

Please be advised that on the Questions and Answers document the answer to Question #3 should read "50% of pediatric patients."

March 29, 2016 - Clarification

Please be advised that there is an error on Page 11, Section IV, 4.2: The correct close date for the application should read May 6, 2016 at 4:30 pm.
