Safe Options Support (SOS) Program: CTI Teams New York City

OMH is soliciting applications to support 2 SOS (Safe Options Support) Critical Time Intervention (CTI) Teams in New York City (NYC).

One team will be awarded in each of the following boroughs:

  • Richmond County (Staten Island)
  • Queens County (The Rockaways area of the borough).

SOS CTI Teams will use an evidence-based CTI approach to provide intensive outreach, engagement, and care coordination services to the estimated ~3,500 street homeless individuals in NYC.

Please be advised that there was an error when creating this RFP in Grants Gateway and as a result, none of the required documents that need to be provided either in response to the budget questions or as part of the application are accessible. Please use the two budget documents, the provider contact form and the Sexual Harassment Certification form found here on this page.

Please be advised that the Budget Template and Budget Narrative Template will be uploaded in response to those two separate questions (6.7a and 6.7b) in the Program Specific Questions Section.

The Sexual Harassment Certification and the Provider Contact Forms will need to be uploaded into the Grantee Document folder.
